I understand that participating in physical exercise and fitness activities carries inherent risks, and I-along with those listed on this form-voluntarily assume all such risks. I acknowledge that the gym fitness center and its staff are not responsible for any injuries or accidents that may occur during my membership. I understand that it is my responsibility to clean equipment after each use, re-rack my weights and put away any equipment used while in the gym. I also authorize Iron Mule Fitness Club to charge my credit/debit card listed for agreed upon purchases. I understand that my information will be saved to file for future transactions on my account.
